Objectives
To assess the readiness for hospital discharge in first-time stroke patients and discharge from the hospital to home only.
Nurse and healthcare providers can use this website to predict unplanned readmissions in stroke patients.

Evaluation

0 - 13 points = Patients no-readiness for hospital discharge and high probability readmission within 30-day
14 – 29 points = Patients readiness for hospital discharge and low probability readmission within 30-day
